We ask that you bring your completed new patient paperwork (below) and your dental insurance information to your first visit. If you prefer to complete the new patient paperwork in the office, please arrive 15 minutes early for your appointment.
You may also want to write down any questions or concerns you have regarding your dental health and bring this list with you. This will help to ensure all your concerns are addressed.

After a thorough exam, your dentist will discuss his findings and all options for treatment (if treatment is needed). Our goal is to help you come to an informed decision regarding care.
One of our main goals is to stay on time for all appointments. You can help us by being on time for your appointment.
Your appointment time is reserved especially for you. If you cannot keep your appointment, please give us at least 24 hours advance notice. If you are scheduled for a surgical appointment, we ask that you give us a minimum of 48 hours notice.
